Our mission is to build a holistic, interdisciplinary care pathway in partnership with our patients. Through personalised support, we help them to manage their health holistically and improve their quality of life, building a therapeutic alliance based on caring and listening in a relationship of trust.

Diabetes

Diabetes is a chronic disease that can lead to cardiovascular complications and damage to the nerves, kidneys, and eyes.

Diabetes is characterised by an abnormal rise in blood sugar levels. Normal fasting blood sugar levels should be less than 5.6 mmol/l. Diabetes is diagnosed when blood sugar levels exceed 7 mmol/l on two occasions. This disease results from an insufficiency or resistance to the action of insulin, an essential hormone for the use of glucose by the body's cells.

Our Diabetes and therapeutic education Center offers personalised care to help diabetic patients find their own strategy for treating and living with the disease.

The Diabetes and therapeutic education Center is only accessible with a medical prescription. Once your general practitioner has sent your health check-up, you will be welcomed by the secretariat.

First appointment with the diabetologist :

This initial contact enables us to establish an assessment, diagnose your specific needs, and lay the foundations for personalised follow-up.

Multidisciplinary support :

Depending on your identified needs, you may benefit from a consultation with our dedicated nurse and/or dietician. Our nurse will monitor your blood sugar levels regularly and offer advice on self-management, while our dietician will work with you to develop an individualised nutritional strategy.

Monitoring and adjustments :

Your therapeutic support is continuously adjusted according to changes in your illness and health. We work closely with your general practitioner to ensure continuity of care.

The team

The interdisciplinary team at the Diabetes and therapeutic education Center consists of a diabetologist and endocrinologist, Dr. H. Delgado Velez, a dedicated nurse, Ms. A. Bernardino, and a dietician, Ms. F. Bigler. The team is completed by a medical secretary, Ms. S. Rolland. Together, their aim is to provide comprehensive support tailored to each individual's specific needs.

Open to all prescribing doctors, the centre offers multidisciplinary expertise to guarantee comprehensive follow-up. Thanks to a collaborative approach, the support offered aims to meet the specific needs of each patient.
